Our Story
PhIX was born from a critical need in Canadian healthcare: the lack of a standardized, secure platform for pharmacy incident reporting. Recognizing that medication errors and near-misses were often underreported due to fear of repercussions, we set out to create a solution that prioritizes learning over blame.
In partnership with ISMP Canada and supported by Pharmasave, PhIX provides Canadian pharmacy professionals with the tools they need to report incidents anonymously, share learnings, and contribute to system-wide improvements in patient safety.
